R5:  Mapping and Modeling RRTC Instruments to the ICF


Why is this study needed?

One aim of the International Classification of Functioning, Disability, and Health (ICF) is to provide researchers and clinicians with a unifying framework for understanding the disablement process. General agreement has been reached that participation is a fundamental outcome of treatments during rehabilitation and of community-based interventions. Participation as depicted in ecological models depends on environmental context and person factors. The ICF provides a framework for classifying the environmental factors involved in multiple categories of participation; person factors are not yet classified within the ICF. Participation is described as engagement in major life activities in the lived environment and as the observable performance within a life situation. The environment is defined as those factors that “make up the physical, social and attitudinal environment in which people live and conduct their lives.“ The relative contributions to the transactional relationships of the person-environment factors fundamental to participation have been the subject of increased attention in community-based rehabilitation research. Consensus is lacking on measures to assess person-environment transactions and their relationship to participation. The challenge is to translate existing function and environmental instruments into ICF-compatible language. These complex interactions are critical to understanding outcomes in real-world situations.

How will this study help patients and rehabilitation stakeholders?

Mapping of PROMIS, NeuroQOL, NIH Toolbox and new items developed and refined as part of this RRTC will be of great value to health and rehabilitation researchers internationally. We expect that our work products will be used by investigators who wish to select items that represent specific ICF components. SEM (structural equation modeling)-identified relationships will help advance our understanding of how ICF components are related.  Results will also inform discussions among scientists and clinicians to target interventions better.

What are the project objectives?

  • Objective 1:  Map items from instruments developed and refined as part of projects R1, R2 and R3 to the ICF.
  • Objective 2:  Model the relationships among ICF domains using items validated in R1, R2 and R3. 

How will the project objectives be achieved?

The goal of this study is to describe relationships between the constructs of health condition, body structure/function, activities/participation and environmental factors, particularly community environments, in people with SCI, TBI and stroke.  The items from the measures developed and evaluated with the R2 and R3 samples will be used as manifest variables to construct several structural equation models.
