Post Acute Care Payment Reform Demonstration (PAC:PRD): Project Implementation and Analysis

Objective

This study will further test the Continuity Assessment Record and Evaluation (CARE) tool and examine differences in costs and outcomes for post-acute care patients of similar case mix who use different types of post-acute care providers.
