Preparing for Your Appointment

If you are new to the world of prosthetics and orthotics, or just new to the Rehabilitation Institute of Chicago (RIC), you may be wondering what to expect during your appointments with us. Prostheses and orthoses are custom made devices that require detailed evaluation, casting, fitting and follow-up care. 

Please follow the following links for an outline of a typical fitting process at the RIC Prosthetics & Orthotics Clinical Center.

Before scheduling your first appointment, we will need to receive a doctor’s order for the device that has been prescribed for you. You may fax this order to us at 312-238-1932 or mail it to us.

Before arriving for your first appointment, feel free to print and complete the following form:

If you prefer, you may wait to read and fill out this form when you arrive at RIC for your appointment. There will be some additional paperwork as well when you arrive. Please plan to arrive 15 minutes early to complete all necessary paperwork before your scheduled appointment. Remember to bring your insurance card to every appointment.
